About the Author

Emma K. O’Donoghue, DClinPsy, is a senior clinical psychologist working in community psychosis settings in a South London National Health Service Trust. She has a long-standing interest in using acceptance and commitment therapy (ACT) approaches with clients experiencing first episode and established psychosis, as well as those with bipolar affective disorder. She has coordinated two randomized controlled trials of ACT workshops for clients and caregivers in community psychosis settings, and clients experiencing bipolar. She is also involved in working with service users to facilitate ACT interventions. Emma regularly trains psychologists in ACT for psychosis interventions, and teaches masters and doctoral courses in ACT approaches.Eric M. J. Morris, PhD, is clinical psychologist and director of the La Trobe University Psychology Clinic in Melbourne, Australia. Eric previously worked as the psychology lead for early intervention for psychosis at the South London and Maudsley NHS Foundation Trust. He has twenty years’ experience treating people with psychosis, and their families, using psychological therapies. Eric completed a PhD at King’s College London, researching ACT as an individual- and group-based intervention for people recovering from psychosis, and as workplace resilience training for mental health workers. An author of forty-one books and more than 575 scientific articles, he has shown in his research how language and thought leads to human suffering. This book will serve as a very effective guide to facilitating recovery oriented groups for people with psychosis, and for family members, using an ACT approach.ACT seems like a great way to start off working with people with psychosis, because it emphasizes seeking out a really crucial balance between two kinds of responses that cause problems.• 1. Being taken over or pushed around by feelings, thoughts, impulses, voices, etc. (which ACT calls “fusion”)• 2. Getting pre-occupied with trying to get rid of feelings, thoughts, impulses, voices, etc. (which ACT calls “experiential avoidance.)Instead of falling into either of these two responses, people are encouraged to take committed action to move in a direction they value – the kind of action that can lead to recovery.The authors note a big goal of theirs was to avoid having any of the group exercises come across as “lame” and they did seem to arrive at a program that is stimulating, focused, down to earth, and quite useful.I do think though that some of the ACT materials could benefit from being tweaked a bit to better set the stage for further steps toward a more complete recovery.For example, a central metaphor used compares disturbing feelings etc. with unwanted passengers on a bus, and it is suggested that instead of letting the passengers control the direction of the bus or to waste time trying to kick them off the bus, that the passengers simply be “taken along for the ride” as the driver focuses on a valued direction (such as being a bus race driver in the example.)A problem with that metaphor is that it proposes a really quite weird relationship between the person’s conscious self, or “bus driver” and the inner experiences that may challenge the self. It seems to suggest just noticing these “passengers” in a superficial way, but then avoiding any curiosity about their origin or empathy about their own wants and needs.A better metaphor in my opinion, used in therapy approaches like Internal Family Systems or Compassion Focused Therapy, frames disturbing feelings etc. as more like family members. This metaphor suggests a richer relationship: there is still a need to avoid being pushed around by the “family members” and to also avoid getting caught up in unproductive attempts to push them away or out, but learning to just “take them along for the ride” may be only a first step. Later one might also notice they have something of value to offer, and one might find meaning in interacting with them and even caring for them. Maybe, a little more like a bus driver that also cares about the needs of the “passengers?”Anyway, other than the need to tweak the metaphors to better set the stage for deeper healing, I think this book and the ACT approach in general offers some great beginning skills in a very accessible format.

This is a great resource for anyone working with psychosis. It is very practical and gives loads of ideas for people new and familiar to ACT. I have also used the book for people without psychosis and have found the exercises really helpful in building my confidence with using ACT. Would definitely recommend!
